Common Variable Immunodeficiency: Overview, symptoms, and treatment

Common Variable Immunodeficiency is an immunodeficiency disorder that occurs frequently. It’s characterized by a reduced ability to make antibodies. This leads to a weakened immune defense, making affected individuals prone to repeated and sometimes severe infections–especially in the ears, sinuses, bronchi, and lungs.

CVID can occur at any age, but it is more commonly diagnosed as an adult in their third or fourth decade. Many cases of CVID are not detected for many years due to their varied presentations. This increases the risk of developing complications, such as Bronchitis.

Causes and risk factors

The cause of CVID is not known in most cases. The condition affects both females and males, and it does not have a clear pattern of inheritance. CVID is one of the most common forms of Primary Immunodeficiency Disease.

Associated Conditions

CVID patients often suffer from more than infections. Many patients with CVID may also develop autoimmune diseases where the immune system attacks healthy cells. Some of the most common issues are:

  • Autoimmune anemia

  • Thrombocytopenia (low platelet count)

  • Neutropenia is a condition in which the white blood cell count is low.

  • Rheumatoid arthritis

The gastrointestinal system can be affected by CVID, causing chronic diarrhea, abdominal discomfort and malabsorption. Granulomas, which are clusters of inflammatory cell, can form in some organs, including the liver, lungs, skin and lymph nodes.

There is also an increased risk of cancer in people with CVID, especially lymphomas.

Recognizing Symptoms of CVID

The most common symptom is recurrent infection. These infections are usually found in:

  • Respiratory System (such as pneumonia, bronchitis and sinusitis)

  • Ears can lead to frequent ear infections

  • Digestive system – including chronic GI infections

Bronchitis can result from chronic lung infections, making them more susceptible to infection and causing a slower recovery.

Diagnosis of CVID

Patients with symptoms such as:

  • History of frequent respiratory infections or ear infections

  • Recurrent sinus infections that don’t fully resolve

  • Unidentified gastro-intestinal complaints

Blood tests are used to measure antibody levels as well as B-lymphocytes which are essential for antibody production. CVID occurs when immunoglobulin (IgG) levels are abnormally low and vaccine responses inadequate.

Treatment and Management of CVID

CVID has a cure is not known but a long-term treatment can improve the quality of life. Immunoglobulin Replace Therapy (IRT),, is the cornerstone of treatment. It involves the regular administration of immunoglobulins (antibodies) via intravenous (IVIG or SCIG) or under-the-skin (SCIG) injections in order to boost your immune system.

Other treatments include:

  • Antibiotics– to treat active infections or prevent frequent infections

  • Immunosuppressive drugs or corticosteroids for the management of autoimmune complications

  • Monitoring for early detection of cancer and organ involvement

For effective management, patients usually require lifelong treatments and constant follow-up by an immunologist.

Living with CVID

The condition is chronic, but many people can live a full life with the right medical care. Early detectionRegular antibody therapy and Infection prevention strategies all play an important role in reducing the health risks and improving day-to-day function. In complex cases, it may be necessary to use a multidisciplinary approach that involves immunologists, lung specialists, and other experts.

Final Thoughts

Common Variable Immunodeficiency (CVID) is a complex disorder of the immune system that requires constant care and attention. Individuals with CVID are able to manage their disease and maintain a high quality of life thanks to advances in immunoglobulin treatment and increased awareness.
